Individuals - Print out an Individual Membership Form and fill it out.
- Mail the following to:

- Your completed membership form
- Photocopies of two pieces of ID, one of which must be government issued (driver's license, passport etc.)
- Check or money order -- see How to Open an Account Page for minimum requirements for different accounts
- Note: There is no need to send a separate check or money order for your one-time $10 membership fee. We will deduct it from your initial deposit.

Arts Organizations

- A list of your organization's Board of Directors
- A copy of your most recent budget and audit
